Community Health Needs & Assets Assessment Survey

The Morehouse School of Medicine Prevention Research Center (MSM PRC) is conducting the 2022 Community Health Needs & Assets Assessment (CHNAA) survey with the purpose of the engaging the local communities of the state of Georgia and identifying their health priority needs. The findings of the assessment will be used to inform future health promotion and evaluation initiatives conducted by MSM. Participation in the assessment involves completing an approximately 20-minute online survey.

The survey will be offered both in English and in Spanish. Participants that are eligible to take the survey will receive a $25 e-gift card upon completion. We value your input and encourage you to share this survey with friends, family, and others within your community.

Eligible participants are:
  • 18 years of age or older
  • Willing and able to provide consent and complete a 20-minute online survey
  • A resident of the Neighborhood Planning Units (NPU): T, V, X, Y or Z
    – OR –
    A resident of one of the following Georgia counties: Appling, Atkinson, Bacon, Baldwin, Bleckley, Calhoun, Candler, Charlton, Clay, Clayton, Clinch, Cobb, Coffee, Colquitt, Crisp, DeKalb, Decatur, Dooly, Dougherty, Elbert, Emanuel, Evans, Fulton, Gilmor, Gordon, Grady, Greene, Gwinnett, Johnson, Laurens, Macon, Montgomery, Polk, Seminole, Stephens, Sumter, Thomas, Tift, Toombs, Troup, Turner, Ware, Washington, Wayne, Whitfield
